Obstructive sleep Apnea (OSA) Monthly Membership with CPAP Rental:
$375/month

Includes:

  • CPAP machine rental, setup and mask fitting.
  • Assistance managing supplies.
  • Ongoing therapy management, adjustments, and troubleshooting.
  • Regular check-ins to ensure effective treatment.

OSA Monthly Membership with Owned CPAP:
$325/month

Includes:

  • All CPAP therapy management services (managing supplies, adjustments, troubleshooting, and check-ins).

OSA or Central Sleep Apnea (CSA) Monthly Membership with BPAP Rental:
$450/month

Includes:

  • BPAP machine rental, setup and mask fitting.
  • Ongoing therapy management, adjustments, and troubleshooting.
  • Regular check-ins to ensure effective treatment.

OSA or CSA Monthly Membership with Owned BPAP:
$400/month

Includes:

  • All CPAP therapy management services (managing supplies, adjustments, troubleshooting, and check-ins).

Home Sleep Test (HST):
$450

Includes:

  • Diagnostic testing for conditions such as sleep apnea. Will include follow up call or telemedicine visit to discuss results and briefly review treatment plan.

Insomnia Treatment Program (CBT-I):
$350/session

Includes:

  • Individualized psychotherapy sessions for insomnia management. Typically, will undergo between 6-10 sessions of about an hour each over a 6-20 week period.

Insomnia Treatment with Medications:
$250/visit

Includes:

  • Targeted medications for specific insomnia disorder.

Restless Leg Syndrome Initial Consultation (RLS):
$350

Includes:

  • Specialized assessment and management planning for RLS. May recommend getting blood work completed to evaluate for iron deficiency.

Restless Leg Syndrome Follow Up:
$250

Includes:

  • Follow up visit to evaluate symptom management, efficacy of medication, and any side effects.

Unusual Sleep Behaviors / Parasomnias Initial Consultation:

Includes:

  • In-depth evaluation and treatment planning for disorders like sleepwalking or night terrors.

Parasomnias Follow Up:

  • Telehealth appointment to discuss treatment response.

Nocturnal Hypoxemia Evaluation and Oxygen Rental:
$450/month

Includes:

  • After completion of home sleep test noting nocturnal hypoxemia or for high altitude sickness. Renting an oxygen concentrator is a great option for both in home concentrators or portable use. Note it is $450 for each device. Also includes nasal cannula.

Delayed Sleep Phase, Early Sleep Phase Disorder of Shift-work Evaluation:
$400

Includes:

  • Evaluation of sleep wake schedule and multi-modal treatment. Will include actigraphy testing for 2 weeks.

Delayed Sleep Phase, Early Sleep Phase Disorder of Shift-work Follow Up:
$350

Includes:

  • Review of sleep diary and actigraphy. Lifestyle recommendations will be made to shift schedule.

Jet Lag Prevention:

(No membership fee required)

Includes:

  • Telehealth appointment with personally tailored recommendations going to and from destination to help prevent/lessen jet lag. Will receive call prior to visit to document destination.

Hypersomnia Treatment:
$350

Includes:

  • Personalized evaluation of sleep diary and sleep study and treatment of excessive daytime sleepiness. Likely will include treatment with stimulants and lifestyle modifications.

Narcolepsy Treatment:
$350

Includes:

  • Will require in laboratory sleep study and Multiple Sleep Latency Test to diagnose Narcolepsy. Treatment of Narcolepsy with stimulants and lifestyle modifications.

High Altitude Sickness Evaluation:
$350
(No membership fee required)

Includes:

  • Includes history and physical examination to evaluate for high altitude sickness. Treatment may include supplemental oxygen therapy. In severe cases, may require hospitalization.
